Advance Medical Directives
As an accredited home medical equipment provider, High Country Oxygen is required to inform you of your rights regarding advance medical directives. Advance medical directives are legal documents that allow you to give directions for your future medical care. They help protect your right to choose by communicating your wishes if you become physically or mentally unable to do so yourself.
Types of Advance Directives
Living Will
A written document that explains your wishes for medical care if you have a terminal condition or are in an irreversible coma and are unable to communicate.
Medical Durable Power of Attorney
A document that allows you to name a person to make medical decisions for you if you become unable to do so. In Colorado, this is governed by the Colorado Patient Autonomy Act.
CPR Directive
A document that instructs emergency medical personnel regarding your wishes about cardiopulmonary resuscitation.

• You are not required to have an advance directive to receive services from High Country Oxygen.
• High Country Oxygen does not create advance directives. We are providing this information so you are aware of your rights.
