Patient Responsibilities
As a client/patient of High Country Oxygen, you have the responsibility to:
1. Provide accurate and complete information about your health, including your medical history, current medications, and any changes in your condition.
2. Follow the plan of care/service as agreed upon with High Country Oxygen and your prescribing physician, including proper use of all equipment provided.
3. Use equipment only as instructed and in accordance with the training and education provided by High Country Oxygen personnel. Do not allow others to use equipment prescribed for your care. 4. Notify High Country Oxygen promptly of any problems, concerns, or changes in your condition, including equipment malfunctions, alarms, or any adverse events.
5. Notify High Country Oxygen of any changes in your address, telephone number, insurance information, or prescribing physician.
6. Treat High Country Oxygen personnel with courtesy, respect, and consideration, and provide a safe environment for personnel who deliver equipment or provide services in your home.
7. Be available or ensure an authorized representative is available at the agreed-upon time for deliveries, pickups, and service appointments.
8. Maintain the equipment in good condition, follow cleaning and care instructions, and protect equipment from theft, loss, or damage. You may be financially responsible for lost, stolen, or damaged equipment.
9. Return all rental equipment to High Country Oxygen upon completion of your rental period or when the equipment is no longer needed, in accordance with the agreed-upon return instructions. 10. Inform High Country Oxygen if you do not understand any aspect of your care, instructions, or equipment operation.
11. Meet your financial obligations to High Country Oxygen in a timely manner and notify the company if you are unable to pay so that financial hardship options may be discussed.
12. Notify High Country Oxygen and your physician if you decide to refuse care or treatment, understanding the potential consequences of that decision.
13. Follow safety precautions as instructed, including oxygen safety guidelines, electrical safety, and home environment safety measures.
